Health Office

  • Health Office

The health assistant provides emergency services to students at school. She administers medication, observes student health needs, gives first aid for injury or sudden illness sustained at school, maintains health records, supervises vision and hearing examinations, and contacts parents when necessary. In case of an accident or sudden illness, the student may report directly to the health office. If not seriously ill, or if the illness occurs during the class period, the student should report to the teacher and obtain a student pass to the health office. The health assistant will call the parent when it is necessary for a student to go home due to illness.

    Immunizations

Arizona Revised Statute 15-872 requires the parent, guardian, or person in loco parentis of a child to provide the school with an immunization history at the time of enrollment. This history includes the month, day, and year for each vaccination. Parents must show proof of immunizations. Children will not be permitted to attend classes until the immunization history has been provided.

    Medication

All medication must be brought to the school by the parent or other parent-designated adult only. Students are not allowed to carry medication to administer to themselves. A physician must authorize all prescription medication. Medication must be in the original container. The container must be clearly labeled. Along with a physician’s written order, there must be parental written permission for each prescription medication. Teachers do not hold or administer medication to students at school unless otherwise determined by a medical physician.

    Emergency Information

It is of vital importance to keep students' emergency card current.  The following information is needed:

    • Current home address and telephone number, or the number of the closest neighbor who will contact the parent.
    • Place of employment and phone number of both parent/guardians.
    • Name of a person who is responsible for the child in the event parents cannot be located immediately in the event of an emergency.

Ailments such as diabetes, epilepsy, cardiac diseases, asthma, and allergies are some of the diseases that should be noted on health records and discussed with the school health assistant at the school.
