• Parents, please fill out this photo release opt out form if you wish to EXCLUDE the sharing of your student's information and images. By taking no action, you are AGREEING to the capturing and sharing of student data and images. If we receive this OPT-OUT form, you are EXCLUDING the sharing of your student information and images. 

    If the Alhambra Elementary School District does not receive this Release Form by your returning this form within the prescribed time, the District will assume that your permission is given to use the directory information as described in the opt out form.
